WEST HAVEN, March 1, 2021 — Connecticut residents 55 and older can now fill out an online form to register with the Vaccine Administration Management System (VAMS), a portal administered by the state Department of Public Health.You will need to enter your full name, birthdate, ZIP code, occupation (including “retired”) and email address. The information you enter will only be used for purposes of scheduling your vaccination and will be kept private.Once registered, VAMS will email and guide you through the appointment process for receiving the COVID-19 vaccine. At this moment, only people eligible under Phase 1a, Phase 1b or Phase 1c who are 55 and older can register.Please only fill out the form if you are eligible to receive the vaccine.Also, Connecticut residents 55 and older can now register with VAMS by phone at 877-918-2224.The line will take calls from 8 a.m. to 4:30 p.m. Monday through Friday and will offer a call-back option when all contact specialists are busy serving other callers. The team will aim to return calls as soon as possible, with the goal of same-day response.Please note that every individual must have their own unique email to be vaccinated with VAMS. For example, if you are registering your elderly parent and use your email, you will not be able to be vaccinated using that same email.
